ALL EXTRAS (MINI-BAR, TELEPHONE, ROOM SERVICE ETC.) MUST BE SETTLED DIRECTLY WITH THE HOTEL UPON DEPARTURE.

BOOKING CONDITIONS: Please complete this form and return to FOCUS ON HEALTH by 20, June 2010. After this deadline requests will be accepted, however hotels rooms are subject to availability and cannot be guaranteed.Deposit of 1 night is requested to confirm your booking.

CANCELLATION/CHANGE OF RESERVATION: Cancellations/changes must be notifi ed in writing (email or fax) to FOCUS ON HEALTH. Cancellations/changes made by phone cannot be accepted. Please do not contact the hotels directly as reservations are held by FOCUS ON HEALTH. For hotel reservations cancelled before 11, July 2010 full deposit will be refunded less EUR 40. After 11, July 2010 deposit of 1 night will NOT be refunded. After 6, August 2010 all nights as reserved will be charged. (FOCUS ON HEALTH holds the rights to charge your credit card for the rest of nights OR not to refund the full pre-payment). In case of no-show or later arrival or earlier departure other than reserved all nights as reserved will be charged.

PAYMENT CONDITIONS:
• 1 night deposit will be to guarantee the booking (not-refundable after 11, July 2010). All bookings must be guaranteed by a valid credit card (VISA, EC/MC). Expiration of the card must exceed 1, October 2010. FOCUS ON HEALTH holds the rights to pass your credit card details to the respective hotel. Final bill (extras) must be settled directly with the hotel upon departure.
• Full pre-payment can be made by a credit card or a bank transfer. (1 night is not refundable after 11, July 2010 and the full prepayment will be charged in case of cancellation after 6, August 2010). Bank transfer will be accepted.Please do not complete the bank transfer before availability of requested room is confirmed. Please include your PRN as variable symbol to identify your payment. A full payment free of any bank charges must be received by FOCUS ON HEALTH. All extras must be settled directly with the hotel upon departure.

Credit card will be charged in Euros (EUR) which may result in slightly higher charge than originally stated depending on your currency rate.

Online payment can be done only for the total amount. If you wish to pay only for one night you have to make bank transfer. In this case the remaining amount has to be transferred by the 6th of August 2010.
